- Provide high-quality nursing care to a diverse patient population including health promotion chronic disease management and preventative care.
- Support patients with a holistic approach addressing both physical and mental health needs.
- Perform clinical duties such as immunizations wound management cervical screening blood tests medication administration and routine health screenings.
- Manage chronic disease conditions such as diabetes asthma and hypertension by offering advice and monitoring health plans.
- Collaborate effectively with GPs other nurses and allied healthcare professionals to deliver coordinated patient care.
- Provide patient education on self-care lifestyle changes and management of health conditions.
- Maintain accurate and up-to-date patient records adhering to confidentiality and data protection policies.
- Participate in quality improvement initiatives and assist in the development of practice policies and protocols.
- Actively engage in professional development and keep up-to-date with nursing best practices and guidelines.
- Registered Nurse (RN) or equivalent with a valid nursing license.
- Previous experience as a Practice Nurse or relevant clinical nursing experience.
- Strong clinical skills including wound care vaccination and chronic disease management.
- Excellent communication and interpersonal skills with a patient-centered approach to care.
- Ability to work collaboratively in a multidisciplinary team as well as independently.
- Proficiency in electronic health records and basic computer literacy.
- Commitment to maintaining patient confidentiality and adhering to professional standards.
- Additional qualifications or certifications in areas such as diabetes management sexual health or smoking cessation are highly desirable but not mandatory.
- Conduct face-to-face phone or virtual consultations with patients across various age groups and health needs.
- Perform and document clinical assessments diagnose minor ailments and provide appropriate treatments or interventions.
- Monitor and follow-up on long-term care and chronic disease management plans to ensure continuity of care.
- Administer vaccinations including childhood immunizations flu shots and travel vaccines.
- Conduct routine health checks such as spirometry ECG and cervical screening.
- Offer lifestyle advice to patients promoting physical and mental well-being.
- Collaborate with other healthcare providers to optimize patient care or refer to specialists as needed.
- Participate in team meetings training sessions and quality improvement projects to enhance practice performance.
- Maintain a welcoming and non-judgemental environment for patients from all walks of life.
