Are you ready to make a meaningful impact in the lives of seniorsEngage Senior Therapy a proud member of Fusion Physical Therapy Partners is seeking a dedicated Physical Therapist to join our team delivering compassionate care at Assisted Living Facilities located in Manchester NH. This is your chance to empower the geriatric and senior population to regain strength mobility and independence while working in a supportive team-oriented environment.
As a Physical Therapist you will:
- Evaluate patients to assess strength balance mobility and functional impairments
- Develop and implement personalized treatment plans that focus on improving mobility reducing pain and enhancing overall function
- Deliver care at one or more assisted living locations providing meaningful services to a diverse group of seniors
- Provide care with a high level of independence in assisted living communities while always supported by the broader Engage team for guidance and resources
- Educate patients and families on home exercise programs and strategies for maintaining functional independence and a better quality of life
- Document patient care services in compliance with state and facility standards using EMR
- Enhance professional growth and development through participation in educational programs in-service meetings and workshops
