Job Summary
We are working closely with a local authority in Nottinghamshireto assist with the appointment of a Personal Support Advisor on a 1-monthcontract highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.
Rate of Pay: 13.90- 17.99 per hour
Summary:
To provide effective brokerage and support services for adults who require short-term or long-term care and support. The postholder will work with individuals families social care and health professionals and care providers to source appropriate care placements and packages in line with assessed care and support needs. The role will contribute to safe and timely hospital discharges ensuring that people receive person-centred care and support while making effective use of available resources.
Responsibilities:
- Source appropriate residential nursing domiciliary and other care and support services for adults based on their assessed needs.
- Support timely and safe hospital discharges by identifying and securing appropriate care and support arrangements.
- Respond effectively to changing priorities and urgent requests particularly where there are risks to an individuals safety or discharge delays.
- Apply knowledge of the Care Act safeguarding requirements and adult social care responsibilities when arranging care and support.
- Ensure confidentiality and information governance requirements are maintained at all times.
Qualifications:
- Knowledge and understanding of the adult social care provider market.
- Knowledge of adult social care legislation including the Care Act safeguarding and relevant financial regulations.
Essentials:
- Experience of working within adult social care health care brokerage or a related environment.
- Experience of sourcing and arranging care and support for adults.
- Experience of negotiating care packages placements or support arrangements with care providers.

- You should have right to work in U.K
- This role requires an Enhanced DBS
