Rate of Pay: 18.82- 24.09 per hour
Summary:
The Personal Advisor plays a vital role within the Leaving Care Service by supporting young people transitioning from care into independent adulthood. The postholder manages a diverse and complex caseload of care leavers including unaccompanied asylum-seeking children young people with unresolved immigration status and those with disabilities.
Responsibilities:
Manage a complex and diverse caseload of care leavers ensuring support is tailored to individual circumstances and statutory responsibilities.
Act as the primary point of contact for allocated young people providing practical advice emotional support and guidance throughout their transition to adulthood.
Assist care leavers in accessing suitable accommodation education employment apprenticeships training health services and financial support.
Prepare clear concise and evidence-based reports assessments and documentation for reviews meetings and statutory processes.
Identify safeguarding concerns assess risks and take appropriate action in accordance with safeguarding policies and procedures.
Qualifications:
GCSEs (or equivalent) including English and Mathematics.
NVQ Level 3 (or equivalent) in Health and Social Care Youth Work Advice and Guidance Community Work or a related field or equivalent relevant experience.
Essentials:
Experience supporting vulnerable children young people or young adults.
Experience undertaking assessments and developing person-centred support plans.
Knowledge of legislation statutory guidance and best practice relating to care leavers and corporate parenting responsibilities.
Understanding of safeguarding children and vulnerable young people.
Knowledge of issues affecting care leavers including housing education employment health finance and independent living.
