Job Summary

3-month contract role with a local authority

Summary


  • This is a 3-month contract role with a local authority aimed at providing high-quality advice guidance advocacy and practical support to young people aged 17½ to 25 who are leaving care. As a Personal Advisor you will manage a caseload of care leavers including complex and high-risk cases ensuring they receive the necessary support to achieve positive outcomes in education employment training accommodation health finances and independent living.

Responsibilities


  • Manage an allocated caseload of care leavers aged 17½ to 25 including vulnerable complex and high-risk cases.
  • Build positive and effective relationships with young people to engage them in planning for their future and achieving their goals.
  • Undertake assessments of need risk strengths and aspirations to inform support planning and interventions.
  • Develop implement monitor and review Pathway Plans in partnership with young people and relevant agencies.
  • Provide information advice guidance and practical support on education employment training accommodation health relationships and independent living skills.

Requirements:


  • Experience:
    • Experience working directly with young people vulnerable adolescents or care-experienced young adults.
    • Experience managing a caseload and delivering individual support plans.
  • Qualifications:
    • Level 3 qualification or above in Social Care Youth Work Education Housing Community Work Health and Social Care or a related discipline.
    • GCSEs (or equivalent) in English and Mathematics.
    • Enhanced DBS clearance.
  • Additional Information:
    • Working hours: 36 hours per week.
    • Driving license required.
    • Location: 34-36 Woodhouse Road North Finchley London N12 0RG United Kingdom.
    • Work pattern: 2-3 days office-based.
    • Bi-weekly schedule.
