Summary
- This is a 3-month contract with a local authority aimed at delivering a high-quality person-centred service to care leavers. The role is crucial in supporting young individuals to achieve positive outcomes and transition successfully into independent adulthood. Responsibilities include assessing needs developing pathway plans coordinating services and promoting the overall wellbeing independence and safeguarding of young people.
Responsibilities
- Deliver tailored support to care leavers to promote independence wellbeing and positive life outcomes.
- Prepare review and implement Pathway Plans in partnership with young people and supervising managers ensuring parallel planning where required.
- Conduct Human Rights Assessments and ensure that young peoples rights are upheld at all times.
- Coordinate multi-agency services and encourage care leavers to access available support.
- Monitor and review care leavers progress ensuring interventions meet assessed needs.
- Maintain accurate up-to-date case records on electronic systems in a timely manner.
- Work holistically with care leavers and their families during transitions including referrals to relevant services.
- Build effective partnerships with parents carers and professionals to safeguard and promote welfare.
- Support access to education training and employment and address barriers to participation.
Essential Experience Required
- Experience working with young people particularly care leavers or vulnerable groups.
- Demonstrable experience in case management and working within safeguarding frameworks.
- Experience of multi-agency working and coordinating services.
- Experience supporting individuals with complex needs such as mental health disabilities or social challenges.
- Knowledge of housing benefits education and employment pathways for young people.
Essential Qualifications Required
- Relevant professional qualification in Social Work Youth Work Health & Social Care or equivalent.
- Evidence of continued professional development.
- Knowledge of relevant legislation including Children Leaving Care Act safeguarding policies and human rights frameworks.
Additional Information
- Working hours: 36 hours per week.
- DBS required.
- Work pattern: 2 days remote 3 days in the office with one week in every 6 weeks fully office-based.
- Bi-weekly schedule.
