Patient Care Coordinator
Job Summary
MedCare Pediatric is seeking a Bilingual Patient Care Coordinatorto support our Pediatric Home Health Therapy team. This role serves as the primary point of contact for families therapists physicians and referral partners after therapy services are established helping ensure exceptional customer service timely communication and efficient coordination of care.
The ideal candidate is highly organized detail-oriented and passionate about providing outstanding service while managing documentation authorizations and daily operational workflows in a fast-paced healthcare environment.
- Serve as the primary contact for bilingual (English/Spanish) families regarding established therapy services.
- Coordinate communication between families therapists physicians referral sources and internal departments.
- Monitor therapy documentation to ensure timely completion and compliance with agency standards.
- Assist with obtaining tracking and managing insurance authorizations and payer requirements.
- Coordinate physician orders plan of care documentation and other required clinical paperwork.
- Maintain accurate patient records within the electronic medical record (EMR) system.
- Support quality improvement initiatives documentation audits and process improvements.
- Identify workflow challenges and collaborate with leadership to improve efficiency and patient access.
- Provide exceptional customer service while ensuring patients receive timely coordinated therapy services.
Required
- Bilingual inEnglish and Spanish(spoken and written).
- Excellent communication organizational and customer service skills.
- Ability to manage multiple priorities in a fast-paced environment.
- Strong attention to detail and problem-solving skills.
- Proficiency with Microsoft Office and computer applications.
Preferred
- Experience in healthcare administration home health therapy services or a medical office.
- Experience with EMR systems insurance authorizations physician orders or medical documentation.
- Knowledge of home health operations and healthcare compliance.
- Timely and accurate documentation coordination
