Occupational Therapist – Hospital Discharge Service

Liquid Personnel is seeking an experienced Occupational Therapist to join a dynamic fastpacedIntegrated Discharge Service (IDS)within Islington Adult Social Services. Youll work as part of a multidisciplinary team of OTs PTs and social workers supporting residents returning home safely and independently from hospital.

What will your responsibilities be

• Screening referralsfor the Discharge to Assess pathway (7day rota; approx. 2/5 days per week once inducted)
• Postdischarge assessmentsto deliver oneoff interventions equipment minor adaptations and manual handling recommendations
• Strengthbased reablement planningto support independence recovery and prevent readmission
• Collaborative workingwith Reablement Social Work teams the Integrated Front Door and First Point of Contact services
• Hybrid working available after induction (homebased documentation community visits)
