Summary

Join a dynamic Leaving Care Service in Lincolnshire that is expanding its team to better support Care Experienced Young People aged 1621. This role offers a meaningful opportunity to assist young individuals in their transition to independent adulthood. As a Personal Adviser you will provide practical guidance emotional support and advocacy empowering young people with the tools and confidence needed to succeed.

Responsibilities

  • Build strong positive relationships with young people and their support networks.
  • Develop and review Pathway Plans using Signs of Safety and Valuing Care frameworks.
  • Support young people in key areas such as:
    • Accommodation
    • Health and wellbeing
    • Education employment and training
    • Finances and life skills
  • Coordinate multi-agency support across housing health and partner services.
  • Conduct statutory face-to-face visits (local and national travel required).
  • Act as Lead Professional where appropriate.
  • Participate in a duty rota and provide support during crisis situations.

Requirements
Requirements:

  • Full UK driving licence and access to a vehicle (national travel required).
  • Experience within a local authority or council setting is highly desirable.
  • Experience supporting young people aged 1625.
  • Background in leaving care youth work social care or a similar setting.
  • Strong understanding of safeguarding risk management and vulnerability.
  • Experience working within multi-agency environments.
  • Proven ability to build effective supportive relationships.

Qualifications

  • NVQ Level 3 (or equivalent) in Health & Social Care Youth Work or a related discipline (desirable).
