Your role and responsibilities
In this role you will:
- Complete assessments and reviews with customers to understand their needs and goals.
- Work directly with individuals to identify personalised outcomes and support them to achieve greater independence.
- Develop positive relationships with customers their families carers and advocates.
- Collaborate with colleagues across health social care and the voluntary sector to provide joined-up support.
- Respond effectively to changing priorities and urgent situations as part of a busy and dynamic team.
- Contribute to the delivery of high-quality services that help people remain safe independent and connected to their communities.
A typical day might involve meeting with customers carrying out reviews liaising with partner organisations and working alongside colleagues to identify the best solutions and support available. Every day brings new opportunities to help people achieve better outcomes and improve their quality of life.

You will need
- Excellent communication and interpersonal skills with the ability to build positive relationships with customers families advocates and professionals.
- The ability to work effectively in a fast-paced environment and contribute to the teams response to urgent work.
- Resilience adaptability and a positive approach when managing challenging situations.
- A commitment to delivering person-centred support and achieving the best outcomes for adults in Suffolk.
