Responsibilities & Duties:

Assists participants with obtaining household goods utilities and other move-in necessities.

Develops individualized housing stabilization plans coordinates admissions and discharges from inpatient treatment.

Collaborates and liaises with law enforcement officers local hospitals medical providers and others in the planning provision and evaluation of these services.

Participates in daily PSH team meetings to discuss client treatment goals and needs.

Collaborate with other members of the PSH Team to provide comprehensive mental health and supportive services to clients.

Provides up-to-date documentation consistent with agency policies and procedures.

Receives clinical and procedural guidance from the PSH Program Manager.

Performs other duties as assigned by the supervisor which are consistent with the position and in compliance with agency policies and procedures.

Bachelors degree in a relevant field with professional clinical experience in the provision of substance abuse treatment or a combination of education training and experience indicating the possession of essential knowledge skills and abilities required.
