Were seeking a dedicated Hospital Liaison Healthcare Assistant to join our compassionate and diverse team in Kings Lynn team- a group of professionals committed to supporting individuals at the Queens Elizabeth Hospital Kings Lynn with compassion.
The post holder will work across the hospital site (e.g. A&E and admissions wards) to provide rapid access to assessment brief interventions/harm minimisation and support discharge planning/onward referrals for people attending hospital with substance misuse related issues.

Where:Kings Lynn (Queen Elizabeth Hospital) Norfolk
Hours: full time 37.5 hours per week
Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles the salary and payments will be pro rata based on contracted hours.
About the role:
- Working closely with your Nursing team to develop pathways to care
- Support service users in opiate and alcohol interventions within a hospital setting.
- You will support on harm reduction interventions along with physical and mental health assessments.
- Keeping the people who use our services at the heart of everything you do
About you:
- Good interpersonal skills and a willingness to work flexibly as part of a team
- Experience of working within substance misuse or a social care setting would be an advantage but not essential
- A clear understanding of the need for and ability to deliver quality services
- Good organisational skills
- Confidence to work on own initiative as well as part of the team
- Communicate confidently and effectively verbally and in writing
- Respond flexibly to the demands of the post
- It is desirable that you have a full UK driving license as travel within Norfolk will be required for this role.
