As part of our multidisciplinary Carers Independent Living Support team youll deliver flexible person-centred support across a range of programs including HACC and carer support services alongside other community-based initiatives.

Youll work with people from diverse backgrounds helping them navigate changing needs stay connected and build independence. This is a dynamic role that combines early intervention practical support and community engagement all with a strong focus on inclusion and wellbeing.

What you will be doing:

  • Building strong trusting relationships with carers clients and families.
  • Providing assessment goal planning and short-term targeted support.
  • Coaching and connecting people with specialist services and community programs.
  • Supporting group activities and community participation.
  • Delivering practical assistance that promotes independence in daily life.
  • Strengthening caring relationships and reducing social isolation.
  • working across multiple programs to provide coordinated holistic support.

About you

A relevant qualification in Social Work Community Services Psychology Counselling OR a related discipline OR demonstrated equivalent knowledge and experience relevant to community services practice.

We are looking for:
