Role Background

Bradford District and Craven Mind is committed to improving mental wellbeing and supporting people experiencing mental health challenges to achieve recovery resilience and fulfilment. As part of our continued commitment to delivering high-quality person-centred mental health support we are working closely with Primary Care Networks General Practitioners and Community Mental Health Teams (CMHTs) across Bradford and Craven to strengthen community-based support for individuals living with Severe Mental Illness (SMI).

The Community Outreach Recovery and Engagement (CORE) Worker plays a vital role in bridging clinical services and community resources ensuring people can access timely holistic support that promotes independence wellbeing and social inclusion. The role has been developed to provide practical and emotional support to individuals who may face barriers to engagement experience long-term mental health difficulties or require assistance navigating local services and opportunities.

Working both independently and as part of a multi-disciplinary network the CORE Practitioner will build strong relationships with service members healthcare professionals and community organisations to help people identify their goals manage risks develop personalised support plans and access the resources needed to sustain recovery and improved quality of life. Through proactive outreach collaborative working and a commitment to equity diversity and inclusion the role contributes directly to Bradford District and Craven Minds mission of empowering people to stay well and thrive within their communities

Role Purpose

In this exciting roleyoullwork closely with both primary and secondary care partnersincluding our dedicated Community Mental Health Team (CMHT) and local GPsto support individuals living with Severe Mental Illness (SMI).Youllbe at the heart of the community empowering people on their recovery journey helping them stay well and connecting them with the support and resources they need to thrive. Role Outcomes

• Provide one-to-one telephone and face-to-face support toservice membersfrom a variety of backgrounds with varying degrees of mental health difficulties.
• Have a good understanding of support services across the districtand able to complete onward referrals to relevant services for people to accessadditionalsupport.
• Complete individual needs assessments riskassessmentsand outcome measures with people we support.
• Develop person-centred support plans to helppeoplestay well.
• Record information electronically andmaintainup to date records.
• Work effectively with colleagues.
• Motivate and encourage people to take steps to achieve positive change in their lives.
• Promote equitydiversityand inclusion through your work.
• Commit to self-development and undertake any trainingrequiredto fulfil your role.
• Participate in individual supervision and internal/external staff development including mandatory training related to the role
• Attend and input to regular team meetings as well as whole organisation engagement days
• Build effective working relationships with Community Mental Health Teamsand attend regular team meetings huddles and other CMHT-related activities including hot desking at their base.

Person Specification

Qualifications

Essential:

• Level 3 or above qualification in a relevant field or at least 2 years relevant experience.
• Must own a car and have a valid drivers license to travel across Bradford and Craven regularly as part of the role.

Experience

Essential:

• Paid or volunteer experience of working with young people or adults in ahealth socialcareorany other settings.
• Experience of supporting people in a clearmeaningfuland non-judgementalway either via telephone or in person (this can include voluntaryexperience)
• Experienced and confident in managing challenging situations andrecognising and responding to safeguarding concerns (this can includevoluntary experience)

Desirable:

• Experienced in working with people who have severe mental illness or long-term complexed need.

Skills and Abilities

Essential:

• Excellent proven communication and organisational skills including an ability to encourage and engage people in ways to make positive changes in their lives.
• Excellent IT skills including use of Microsoft Packages
• Ability to work as part of a team and work alone and use own initiative
• Ability to gather relevant information to support and assess risks

Knowledge

Essential:

• Understanding of the key issues within mental health and the potential
• challenges that people may face
• Knowledge of other relevant health & social care organisations across theBradford District & Craven
• Knowledge of risk management and health and safety issues
• Understanding of safeguarding and good practice
• Understanding of confidentiality and data protection
