Job Summary

3-month contract role with a local authority
Summary
  • This is a 3-month contract role with a local authority designed to provide high-quality person-centred case management services to adults with care and support needs their carers and families. The post holder will play a crucial role in promoting independence wellbeing and positive outcomes through strengths-based assessments support planning reviews and care coordination.
Responsibilities
  • Manage a caseload of adults with a range of care and support needs.
  • Undertake strengths-based assessments reviews and support planning in accordance with statutory responsibilities and best practice.
  • Identify individual strengths assets goals and support networks to promote independence and wellbeing.
  • Monitor and review support arrangements to ensure services remain effective and meet changing needs.
  • Develop and implement person-centred support plans that maximise independence and choice.
Requirements:
Essential Experience Required
  • Experience working with adults within social care healthcare supported living community services or the voluntary sector.
  • Experience managing caseloads and supporting individuals with varying levels of need.
  • Experience carrying out assessments support planning reviews or coordinating services.
Essential Qualifications Required
  • Level 3 Qualification in Health and Social Care or equivalent.
  • Alternatively willingness to work towards achieving a Level 3 Health and Social Care qualification.
Additional Information
  • Working hours: 35 hours per week.
  • Location: 52d Drayton Park London Islington N5 1NS United Kingdom.
  • Enhanced DBS required.
  • Work pattern: Minimum of 3 days in the office and 2 days working from home.
  • Schedule: Bi-weekly.
