Job Summary

We are working closely with a local authority in Bromleyto assist with the appointment of a Care Management Assistant on a 3-months contract highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Rate of Pay: 18.03- 22.47 per hour

Summary:

Bromley seeking experienced and motivated Care Management Assistants / Social Care Practitioners to join the Adult Social Care Coordination and Review Team at the London Borough of under direction and guidance the successful candidate will carry out planned Care Act assessments and annual reviews for adults receiving support in their own homes Extra Care Housing supported accommodation Shared Lives schemes residential and nursing placements and other commissioned care settings.

Responsibilities:

  • Undertake timely proportionate and person-centred assessments of adults and carers in accordance with the Care Act 2014 relevant legislation local policies and procedures.
  • Review the needs and support arrangements of individuals living in their own homes Extra Care Housing supported housing Shared Lives schemes residential and nursing care placements.
  • Support adults and carers to exercise choice and control over their care and support including promoting the use of Direct Payments where appropriate.
  • Support service transitions changes in care provision provider changes and where appropriate hospital discharge arrangements.

Qualifications:

  • A relevant Health and Social Care qualification is desirable but not essential for appointment at BR8.
  • Candidates should have a minimum standard of English equivalent to GCSE Grade AC / Grade 49.
  • Progression to BR9 may require successful completion of a relevant qualification such as NVQ Level 3 or the ability to demonstrate the appropriate level of skills knowledge and experience.

Essentials:

  • Experience of working in a Health and Social Care or Adult Social Care setting.
  • Experience of supporting adults and carers with assessment care planning review or care management activities.
  • Experience of working with adults receiving care and support services in community residential nursing supported housing or similar settings.

  • You should be available to work immediately or at a short notice.
  • You should have right to work in U.K
  • This role requires an Enhanced DBS
