Aftercare Recovery Worker
Job Summary
- Support the service user to connect with activities support to sustainrecovery and reach their goals set out in their aftercare plan e.g. trainingemployment support.
- Facilitate post-treatment groups across the Bi Borough in community settingsoutside of CGL hubs to support individuals in progressing their recoveryjourney independently of services.
- Provide service users with information on options available them within theservice and in their communities.
- Collaborate with the Partnership Lead toengage partner agencies that service users can access for support following
treatment. - Build positive relationships with aftercare providers to ensure recovery support
is available to support post-Rehab early abstinence i.e. Build on Belief (BOB)
Fellowship Groups Aspire2be SMART - Work collaboratively with the Service User Inclusion Worker to understand post
treatment needs ensuring the voices and preferences of service users are
heard and reflected. - Working with Recovery Coordinators Peer Mentors/Volunteers to provide Recovery Check-Ins
- Excellent knowledge of inpatient detox andresidential services
- Experience of working within multidisciplinary teamsand with fostering positive relationships with partneragencies and external providers
- Experience of working with people at all stages oftheir treatment journey and an in-depth knowledgeof the importance that groupwork and aftercaresupport offers
- Excellent written and verbal communication skills andthe ability to provide effective advocacy whenneeded
