At Minehead Medical Centre we are looking for a dedicated Advanced Clinical Practitioner to join our multidisciplinary team delivering high-quality patient-centred care to our local community.

Your Role & Responsibilities

As an Advanced Clinical Practitioner you will work both independently and collaboratively within our multidisciplinary clinical team using your advanced clinical and critical-thinking skills to assess diagnose treat and manage patients presenting with a wide range of conditions.

Your day-to-day will include:

  • Autonomous Clinical Practice: Assessing diagnosing treating and managing patients presenting with undifferentiated and complex health conditions.
  • First Contact Care: Managing acute presentations routine appointments and telephone or remote consultations.
  • Prescribing: Safely prescribing and reviewing medication as an Independent Prescriber in line with current legislation and clinical guidelines.
  • Long-Term Condition Management: Supporting patients with acute and chronic conditions including self-management and lifestyle modification.
  • Urgent Care: Recognising and managing urgent or emergency presentations and arranging appropriate referrals where required.
  • Collaborative Working: Working closely with the wider multidisciplinary team to deliver coordinated patient-centred care.
  • Clinical Governance: Contributing to audit clinical governance and quality improvement initiatives to support safe and effective service delivery.

What Were Looking For

  • Registration: Current registration with a UK professional body such as the NMC HCPC or GPhC.
  • Advanced Practice Qualification: MSc in Advanced Clinical Practice or an equivalent qualification meeting the national ACP framework.
  • Independent Prescribing: Independent/Non-Medical Prescriber qualification.
  • Clinical Experience: Significant post-registration clinical experience in primary care or a relevant setting.
  • Advanced Clinical Skills: Strong assessment diagnostic and clinical decision-making capabilities.
  • Life Support: Current Life Support qualification(s).
  • Teamwork: Ability to practise autonomously while working effectively within a multidisciplinary team.
  • Clinical Governance: Good understanding of evidence-based practice clinical governance and quality improvement.
