LPN Home Health

Seasoned Recruitment is currently seeking compassionate and skilled LPNs to join our Home Health team. As an LPN in Home Health you will play a vital role in providing highquality and personalized care to patients in the comfort of their own homes.

In this role you will be responsible for assisting patients with their daily living activities administering medications performing wound care monitoring vital signs and providing emotional support to patients and their families. You will work closely with a multidisciplinary team to develop and implement individualized care plans to ensure the best possible outcomes for our patients.

Requirements:

To be considered for this position you must meet the following requirements:

• Hold a valid and unrestricted LPN license in the state you will be working
• Have a minimum of 1 year of experience as an LPN preferably in a home health or community health setting
• Possess exceptional clinical skills and knowledge of nursing principles and practices
• Demonstrate strong communication and interpersonal skills
• Be selfmotivated organized and able to work independently
• Have a reliable means of transportation and a valid drivers license
• Be willing to travel within the designated service area
