Requirement description : ALL OF THE FOLLOWING ARE REQUIRED AT NAME CLEAR UNLESS NOTED OTHERWISE.

On Call Requirements : No call
- Practice Setting: Laborist for hospital only
- Reason for Coverage: vacincies
- Dates of Coverage: ASAP
- Total Number of Beds: 300
- Number of Physicians in Practice: 5
- Apps: 12
- Support Staff: nurse midwife on floor RNs
- Onsite Lab Yes
- Onsite Ultrasound Yes
- What Percentage of Practice is Low Risk Pregnancy: 70
- What Percentage of Practice is High Risk Pregnancy: 30
- What Percentage of Patients are CSection: 40
- Average Number of Nursery Admissions Per Day: 45
- Nursery Level (I II III): II
- Documentation System/EMR: EPIC
- Trauma Level/Designation: level II trauma
- Will reimburse Flight Hotel Rental Car or Mileage Billed at the IRS Standard Rate
- Estimated Credentialing Timeframe: 90 days

- Board Certified Required

- 25k rate cap for conversion rate
