Position Overview:

Navitas Healthcare LLC is seeking Licensed Master Social Worker (LMSW) for an exciting Travel or Local job in New York NY.

Key Responsibilities

• Conduct comprehensive psychosocial assessments for hospitalized patients
• Provide supportive counseling and emotional support to patients and families
• Develop and coordinate discharge plans to ensure safe and effective patient transitions
• Arrange appropriate referrals to community resources rehabilitation services and support programs
• Collaborate with physicians nurses case managers and interdisciplinary care teams
• Document assessments interventions and discharge plans in accordance with hospital standards
• Support patients in coping with illness hospitalization and recovery challenges
• Ensure continuity of care through effective coordination and communication
• Maintain compliance with hospital policies and healthcare regulations

Required Qualifications

• Masters Degree in Social Work (MSW) required
• Active Licensed Master Social Worker (LMSW) license issued by NYSED required
• Minimum of 1 year of hospital or inpatient social work experience required
• Experience in medical surgical or rehabilitation settings preferred
• Strong communication assessment and case management skills
• Ability to work effectively in a fast-paced hospital environment
• Excellent documentation and organizational abilities
• Compassionate and patient-centered approach to care
