$3500 Sign-On Bonus for New Hires! (paid in 2 installments at 3 months and 6 months with successful completion of Orientation)

Salary:$22.56-$25.44 Hourly

Schedule: Monday - Friday 8:00AM-4:00 PM (40 hr/week) *ability to be flexible with schedule is highly preferred*



Essential Job Functions:


The Person-Centered Plan Manager also known as a case manager works directly with the individual in the community to help them achieve their goals. Typically approximately 70% of time is spent in the community.

Connects the individual with the services they need to maintain and expand their independence. This may include community services government-funded benefits or other publicly available resources.

Advocates for the individual ensuring that they receive the proper support for their situation.

Completes administrative tasks on behalf of the individual including consultations documentation into an electronic record system benefits eligible forms and other related tasks.

Qualifications:

Associate degree or equivalent experience preferably in a related field.

Valid drivers license with an acceptable driving record and ability to pass background checks.

Basic computer skills including Microsoft Office email communication internet and documentation entry.

Ability to work independently and collaboratively as part of a team.

Compassion resourceful and a strong desire to learn and grow.
