Role Scope:
This is an opportunity for a mission-drivenleader who is passionate about attending to the daily living and safety needs ofPACEparticipants. The PCA Supervisor at the PACE Center oversees the daily operations of the Personal Care Attendants (PCAs)whoprovideservicesinthePACE center in specialty care settingsand in the position ensures high-quality person-centered care in alignment with PACE standards and regulatory guidelines. The PCASupervisorprovides scheduling supervision performance evaluation and support for PCA staff while working collaboratively with the interdisciplinary team members topromote participant independence well-being andensureparticipants physical socialemotionaland recreational needsare met.

- Continue to raise the sharefeedback andhelp us implement changesin order toimprove clinical outcomes and experience for participants.
- Exhibit and honor Habitats values.
- Collaborate with the Center Manager to ensureonboarding training and mentoring of PCA staffin order todeliverhigh-quality care.
- Superviseand manage a team ofPCAs ensuringtimelyand effective delivery of personal care services.
- Oversee PCAperformancemanagementand provide feedback as necessaryto ensure competency and compliance including any disciplinaryactions and performance evaluations for PCA staff.
- Provide supervision support and coaching to PCA staff on the Day Center floor during delivery of services.
- Develop andmaintainstaffing schedules andassignment ofduties.
- Collaborate with the interdisciplinary team (IDT) to assess participant needs and update care plans.
- Support a culture of safetydignityand respect for all PACE participantsand staff.
- Ensurestimelycompletion of all PCA staff documentation including progress notes grievances and SDRs aligned with regulatory timeline requirements.
- Ordersupplies to support with PCA staff daily duties
- Meetsparticipantspersonal hygiene and basic needs ina timelyand caring manner.
- As needed willing to step in and support team with direct care including:
- Assistsparticipants with activities of daily living such as feeding ambulation and other activities.
- Maintains clinical and age specific competencies as defined for the assigned.
- Collects data andrecordsactivity and participant care information on appropriate information isaccuratetimelyand recorded according to established guidelines.
- Provides participant instruction related to safety factors conducive to their care plan and recovery;demonstratesappropriate health safety and aseptic practices.
- Onoccasion provideshomemaking servicesin the homeincludingpreparing and serving meals and snacks according to specified nutritional needs and dietary restrictions as outlined on the plan of light housekeeping tasks as outlinedon planof care including vacuuming mopping dusting taking out trash washingdishesand doing laundry.
- On occasionaccompanyparticipantsfor outside appointments.
- Performs related duties as assigned.
Required Qualifications:
- Completion of accreditedcertifiednursing assistant(CNA)certification.
- High School diploma/GED
- Knowledge ofPACEprogram.
- Ability to work without close supervision or professional guidance and to exercise independent judgment.
- Knowledge of physical mental and social needs of frail older adults.
- Able to manage changing priorities per participant needs.
- Effective interpersonal skills to work with participants caregivers and staff.
- Ability to work sensitively and effectively with individuals of diverse ethnic and cultural backgrounds.
- Ability to be self-directed and able to work independently.
- Requires physical strength to perform essential functions of the job ( with mobility challenges).
- A minimum oftwoyearexperience working with the frail or elderly.
- A minimum of two yearsin a supervisory position.
- Proof of medical clearance for communicable diseases including a TB test.
- Proof of all immunizationsareup to date.
- Proof of current CPR/BLS certification required or requirement to obtain within30 daysofemployment.
- Proof of valid CA drivers license personal transportationgood drivingrecordand auto insurance as required by State law.
Preferred Qualifications:
- Bilingual: Spanish/Mandarin/Cantonese preferred.
- A state issued drivers license personal transportation and auto insurance as required by law.

Compensation:
We take into account an individuals qualifications skillset and experience in role is eligible for medical/dental/vision insurance short and long-term disability life insurance flexible spending accounts 401(k) savings paid time off and company-paid holidays. The expected salary range for this position is$66000 - $75000 and isbonus actual offer will be at the companys sole discretion anddeterminedby relevant business considerations including the final candidates qualifications years of experience skillset and geographic location.
