Job Overview:
We are seeking a Licensed Master Social Worker (LMSW) to provide comprehensive psychosocial support services in an inpatient hospital setting. This role involves patient assessments supportive counseling discharge planning and coordination of care to ensure smooth transitions and optimal patient outcomes.
Key Responsibilities:
- Conduct psychosocial assessments for patients and their families
- Provide supportive therapy and emotional support
- Coordinate discharge planning and continuity of care
- Facilitate referrals to community resources and support services
- Collaborate with interdisciplinary healthcare teams
- Maintain accurate and timely clinical documentation
- Support patients throughout hospitalization and transition planning
- Ensure adherence to hospital policies and social work standards
Required Qualifications:
- Masters Degree in Social Work (MSW)
- Active Licensed Master Social Worker (LMSW) license (New York State)
Experience & Skills:
- Minimum 1 year of LMSW experience required
- Hospital experience preferred
- Experience in inpatient medicine surgery (med/surg) or rehabilitation settings preferred
- Strong psychosocial assessment and counseling skills
- Experience in discharge planning and referral coordination
- Excellent communication and documentation skills
