Location: Dayton OH (Corporate Office On-site)
Schedule: MondayFriday Full-Time

We are seeking a motivated and detail-oriented Central Intake Coordinator to join our corporate team at a growing senior care organization specializing in Skilled Nursing Memory Care and Assisted Living.

This is a fast-paced high-impact role responsible for managing and streamlining the admissions process across multiple facilities.

What Youll Do

In this role you will:

• Manage and coordinate incoming referrals from hospitals case managers and families
• Review referrals for clinical appropriateness payer source and bed availability
• Route and place referrals to the appropriate facility within the organization
• Communicate with hospital partners and facility teams to ensure timely admissions
• Verify insurance coverage (Medicare Medicaid Managed Care Private Pay)
• Support census development and help maximize occupancy across buildings
• Ensure all admission documentation is complete and compliant
• Work closely with facility Admissions teams DONs and Corporate leadership
• Help streamline and improve the overall intake and admissions process

What Were Looking For

• Experience in Senior Care Healthcare Admissions or Hospital Case Management preferred
• Strong understanding of Medicare Medicaid and Managed Care (preferred)
• Excellent communication and follow-up skills
• Ability to work in a fast-paced high-volume environment
• Strong attention to detail and organizational skills
• EMR experience (PointClickCare or similar preferred)
• Team player with a sense of urgency and accountability
