Job Summary
A hospital-based healthcare facility is seeking an experienced Licensed Master Social Worker (LMSW) to support inpatient medicine Med/Surg and rehabilitation patients. The LMSW will provide psychosocial assessments supportive counseling discharge planning and care coordination in a collaborative clinical environment.
Responsibilities
- Conduct psychosocial assessments and supportive therapy
- Coordinate discharge planning and community referrals
- Work closely with physicians nurses and care teams
- Maintain accurate patient documentation
- Support patients and families throughout treatment
Requirements
- Active NYS LMSW License required
- Masters Degree in Social Work required
- Minimum 1 year of LMSW experience required
- Hospital experience required
- Med/Surg or rehab experience preferred
- Strong English communication skills required
