Per Diem Physical Therapist (PT)

Human Touch Home Health is hiring a licensed Physical Therapist to provide home health services throughout Carbon County Pennsylvania with a primary focus on Jim Thorpe Lehighton and surrounding communities.

What Youll Do

• Complete physical therapy evaluations reassessments and discharge planning
• Develop individualized treatment plans and provide skilled therapy services in patients homes
• Help patients improve strength balance mobility and independence
• Coordinate care with physicians and the interdisciplinary team
• Complete documentation accurately and on time

Requirements

• Active Pennsylvania Physical Therapist license
• Valid drivers license and reliable transportation
