Occupational Therapist
Location: Tuba City AZ
Pay Rate: $55.00 $60.00/hr
Schedule: Monday Friday 8:00 AM 5:00 PM
Contract: 13 weeks
Experience: Minimum 2 years
About the Role
We are hiring an experienced Occupational Therapist for a 13-week contract in Tuba City AZ. This role supports inpatient and outpatient rehabilitation services for patients from birth through geriatric age groups with a strong focus on orthopedic care and helping patients improve independence in daily activities.
Responsibilities
- Assess patients and develop individualized occupational therapy treatment plans
- Provide therapy for developmental cognitive emotional physical and orthopedic needs
- Support activities of daily living functional independence and adaptive equipment training
- Provide specialty services such as custom orthoses sensory integration cognitive treatment and DME support
- Monitor patient progress and adjust therapy plans as needed
- Collaborate with physicians nurses social workers and rehabilitation team members
- Complete accurate documentation in the electronic health record
- Support patient education staff training safety and quality improvement activities
Requirements
- Bachelors degree in Occupational Therapy from an accredited program required
- Active Occupational Therapist license required
- Arizona OT license or eligibility for Arizona licensure required
- Minimum 2 years of Occupational Therapist experience required
- Strong orthopedic skills required
- BLS/BCLS certification required
- Inpatient and outpatient experience preferred
- Allscripts experience preferred
- Ability to work Monday Friday day shift
