Job Summary
JOB SUMMARY:
This position provides a consistent and direct point of contact for residents and their families to ensure an open line of communication promoting optimal care and identifying areas of need. Promoting and ensuring a high level of satisfaction and quality outcomes. Work directly with Maplewood and Palmer house administrators on addressing and resolving concerns and grievances
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
A. Acts as the liaison between the resident/family liaison at time of admission and throughout their stay
B. Review all admission information designated policies and Resident Rights and Responsibility with new resident. Resident Rights Grievance process review of admission and overall environment who they can talk to -
C. Meets with both the nursing home and assisted living to discuss any concerns regarding resident care family matters dietary needs laundry issues etc.
D. Collaborate with IDT to improve resident process and success of outcomes.
E. Drives Ambassador/Caring Partner Rounds reimplements and or implements at the AL as well
F. Daily rounds
G. Actively participate in Quality Assurance committees and initiatives
H. Develop and maintain an information service library. Resources for residents and families
I. Meets with both the nursing home and assisted living to discuss any concerns regarding resident care family matters dietary needs laundry issues etc.
J. Communicates and reports daily to nursing home administration and or AL administrator on issues that are reported
K. Research issues that are reported and facilitate resolutions when possible.
L. Education to staff on hire and throughout year ad hoc and formal resident rights/abuse neglect/dementia training
M. Works with nursing home administration on plans of correction
N. Attends Resident Council meetings.
O. Coordinates family council meetings. AL and SNF
P. Meets with and assists residents with completion of satisfaction surveys; (New process)
Q. May assist with resident discharge planning transition within the community as well as outside.
R. May assist with investigating resident issues as directed by nursing home or AL administration.
S. Discharge Satisfaction Surveys.
T. Follow up phone calls for all discharges.
U. Greet all new admissions on the day of arrival and or within 24 hours of admission.
SUPERVISORY RESPONSIBILITIES:
QUALIFICATIONS:
Possession of an Associate Degree in Human Services or related degree and one (1) year of experience involving interviewing and counseling members of the public; OR B) Three (3) years of experience involving interviewing and counseling members of the public; OR C) An equivalent combination of training and experience as defined by the limits of A)
EDUCATION and/or EXPERIENCE
Social work or health care admin/business related degree. Six months to 1yr experience with knowledge of Medicare Medicaid and other insurance products needed.
