Location: South Dennis MA
Shift: Day Shift 5x8s 40 Hours/Week
Contract Duration: 13-Week Contract
Job Overview
A healthcare organization in Massachusetts is hiring an experienced RN Hospice Case Manager for a travel assignment in the South Dennis MA. This role involves providing hospice nursing care and case management services to patients in homes and healthcare facilities.
Responsibilities
- Provide hospice nursing care and case management
- Complete patient assessments and monitor condition changes
- Develop and follow patient care plans
- Coordinate care with physicians and hospice team members
- Educate and support patients and families
- Complete admissions charting and documentation on time
- Supervise Hospice Aides as needed
- Maintain compliance with hospice and nursing standards
Requirements
- Active Massachusetts RN License
- Minimum 2 years of Hospice RN Case Management experience
- Hospice Home Care experience required
- Strong communication and documentation skills
- Ability to work independently
Preferred
- IV Certification
- BSN
