The Ace Consulting Group is partnering with one of our valued clients a nonprofit organization in Chicago Illinois seeking a dedicated Case Manager Housing to support individuals and families through housing and supportive service programs.

The Case Manager Housing will provide direct case management services advocate for client needs and work closely with community partners to ensure clients receive the support necessary to maintain stable housing.

Key Responsibilities

Provide case management and supportive services to individuals and families participating in housing programs
Develop individualized service plans and monitor client progress toward goals
Conduct client assessments including psycho-social and needs assessments
Connect clients with community resources including financial assistance education job training and healthcare services
Maintain accurate client documentation files and program databases
Advocate for clients with landlords community partners and service providers
Facilitate support groups or client meetings as required by the program
Participate in community outreach and collaboration efforts
Prepare reports and documentation in accordance with agency and regulatory standards
Meet regularly with clients to review progress and adjust service plans as needed

What You Bring

Masters degree in Social Work Counseling or a related social services field preferred
Bachelors degree in social services or a related field may be considered depending on experience
13 years of case management or social services experience
Experience working with housing programs vulnerable populations or community support services preferred
Strong communication documentation and client advocacy skills
Ability to assess client needs and develop effective service plans
Valid drivers license and reliable transportation may be required

Schedule & Pay

Schedule: 35 hours per week
Location: Chicago Illinois
Pay: $23 $24 per hour
Type: Direct Hire
