Our main services are in Wellington Dubbo Western Sydney Penrith Nepean and Blue Mountains and we provide outreach services to other towns and communities through our regional programs.

About the Role:

We are seeking a passionate and experienced Chronic Disease Nurse to join our clinical team in Wellington.

In this role you will provide high-quality culturally safe patient-centred care to Aboriginal and Torres Strait Islander clients living with chronic conditions. Youll work closely with GPs Aboriginal Health Workers and a multidisciplinary team to improve health outcomes and support long-term self-management.

Key Responsibilities:

  • Deliver chronic disease management including care planning health assessments and follow-ups
  • Support clients with conditions such as diabetes cardiovascular and respiratory disease
  • Work collaboratively with GPs and Aboriginal Health Practitioners
  • Provide culturally appropriate education and support to clients and families
  • Coordinate case conferences and referrals to allied health and specialists
  • Deliver community-based health promotion programs
  • Participate in outreach services to improve access to care
  • Maintain accurate clinical documentation and contribute to quality improvement initiatives

Essential Criteria:

  • AHPRA Registered Nurse (NSW) with minimum 3 years post-graduate experience
  • Experience in chronic disease management care planning and follow-up
  • Strong understanding of health issues impacting Aboriginal and Torres Strait Islander peoples
  • Excellent communication organisational and time management skills
  • Ability to work both independently and within a multidisciplinary team
  • Demonstrated cultural awareness and sensitivity

Mandatory Compliance Requirements:

  • Current Working with Children Check
  • National Police Check
  • NSW Drivers Licence
  • First Aid & CPR Certificate
  • COVID-19 vaccination (minimum 2 doses)
  • Up-to-date immunisation records
