Role overview:

  • Support children young people families and carers as they engage with the Stride Kids service helping create a welcoming connected and family-inclusive experience.
  • Work alongside families to support engagement with the service assist with navigating referral pathways and community supports and reduce barriers to accessing care.
  • Provide practical emotional and systems-based support to families and carers including advocacy service coordination and linking with relevant supports where appropriate.
  • Support intake engagement and transition processes for children young people and families entering or exiting the service.
  • Develop and maintain collaborative relationships with clinicians schools community organisations and other stakeholders to support coordinated care.
  • Attend and contribute to community and interagency meetings on behalf of the service to strengthen partnerships and referral pathways.
  • Respectfully and effectively respond to issues raised by clients carers family members and community stakeholders.
  • Maintain accurate documentation and contribute to a positive safe and supportive team environment.


Requirements:

  • Minimum of a Diploma in mental health or community services related discipline (ideally a completed Degree)
  • Previous experience in a Support Worker or related role
  • In- depth understanding of issues pertaining to mental illness/or disabilities and their impact on individuals families and communities.
  • Background knowledge and understanding of drugs alcohol and illicit drug use impacting upon the health of individuals and families
  • Experience ingroup facilitation
  • The ability to work autonomously under supervision and as part of a team.
  • Demonstrated effective listening empathy and communication skills
  • Excellent negotiation problem solving and decision making skills as well as the ability to work to time frames meet deadlines and work in challenging situations.
  • Demonstrated ability to work with families carers government agencies and community services
  • Willingness to abide by the Disability Services Acts and Standards and Work Health & Safety Act
  • Competence with Microsoft Office
  • Must be eligible to apply/currently hold a Blue Card
  • Must hold/be willing to apply for NDIS worker screening clearance
  • Current Drivers license
