DEPARTMENT SUMMARY: The Los Angeles LGBT Centers Department of Health Services is one of the nations largest and most experienced providers of LGBTQ-specialized primary care and related services Our mission to help LGBTQ people lead healthier happier lives is supported by our comprehensive array of client-centered services including primary care mental health psychiatry sexual health pharmacy case management nutrition clinical research housing services and a food pantry. The Center is one of a handful of Federally Qualified Health Centers in the nation specializing in serving LGBTQ people.
JOB SUMMARY: Under the supervision of the Director Nursing & Case Management Services the Manager Nursing Case Management (RN) provides direct supervision and oversight of the RN Care Managers the Manager Nursing Case Management (LVN) and the Audre Lorde Health Program (ALHP) RN Manager. The Manager Nursing Case Management (RN) ensures that Case Management Services operates with-in industry standards and meets all contractual requirements and guidelines.

QUALIFICATIONS
- Minimum of two years experience in direct patient care preferably in an ambulatory care setting
- Minimum of two years experience in Case Management preferred
- Minimum of two years nursing management experience in out-patient setting
- Experience in HIV/AIDS other infectious diseases and chronic disease care/management
- Basic knowledge of state/federal reporting requirements pertaining to infectious diseases
- Knowledgeof budget processes quality assurance/improvement distribution reports DHSP HRSA FQHC PCMH standardsand other regulatory standards including state/federal laws rules and regulations
- Strong communication organizational and interpersonal skills with a high level of attention to detail
- Proficiency withMicrosoft Office Electronic Health Records and other software systems
- Ability to be self-motivated and dedicated to initiating and completing complex tasks
- Ability to handle multiple tasks be detailed oriented and maintain strict confidentiality of medical information
- Able to workwell independently with minimum supervision
- Bilingual (English/Spanish) skills a plus
- Knowledge of or experience working with the LGBTQ community and familiarity with issues of relevance to LGBTQ people
- A passion for the Centers work and its mission to make the world a better place for LGBTQ people
- Demonstrated ability to work effectively with people of diverse races ethnicities nationalities sexual orientations gender identities gender expression socio-economic backgrounds religions ages English-speaking abilities immigration status and physical abilities in a multicultural environment.
TRAINING EDUCATION AND LICENSURE
- Current California RN license
- Current AHA CPR certification
- BSN or MSN Preferred
- Current and Active ACRN Certification preferred
