As Engage Program Manager youll contribute to our success in the following ways:

Key Accountabilities:

  • Leads and manages a team of staff to ensure effective coordination of patient care across disciplines.

  • Directs the planning implementation and evaluation of quality patient care for SHH patients.

  • Fosters consistent communication with department leaders and direct reports to support collaboration and alignment of priorities.

  • Builds visibility and accountability across the organization strengthening relationships with internal and external stakeholders.

  • Represents the department on committees and in special projects contributing to organizational strategy and decision-making.

  • Provides oversight and guidance to clinical teams ensuring access to appropriate resources and support in alignment with scope of practice requirements.

  • Reviews and interprets patient/family health data (physical psychological and social factors) to inform clinical team decision-making.

  • Aligns customer needs with appropriate internal resources including skilled care personal care alert follow-up and other healthcare services.

  • Ensures compliance with federal state and local regulations; monitors and implements changes as required.

  • Establishes and maintains efficient systems and communication channels to support continuity and quality of care.

  • Evaluates agency operations and systems identifying opportunities to improve workflow productivity and resource allocation.

  • Performs other duties/projects as assigned.

What is Required

  • Graduate from an accredited RN program.

  • 2 years experience within a home health setting is required field experience preferred

  • Prior management experience is required.

  • Proficiency with MS Office Suite (Word Excel Outlook etc.)

  • Demonstrated knowledge of the healthcare industry and telehealth.

  • Experience in surgical nursing critical care and experience with Homecare Homebase EMR are preferred.

  • Must have current drivers license reliable transportation and insurance in accordance with the laws of the state.

  • Must be able to sit stand bend lift and move intermittently and be able to lift at least 25lbs
