THE ROLE:

We are seeking an Occupational Therapist for a community based aged-care role.

As an Occupational Therapist youll play a pivotal role in enhancing the independence wellbeing and quality of life of your clients. As a part of this you will conduct in-home assessments and provide clinical interventions including the recommendation of assistive technology and basic home

We are looking for an Occupational Therapist who can cover parts of the Perth Metro area (northern and southern suburbs). This is a hybrid role working with clients in their homes in our office and from home.

Duties will include:

  • Provide expert advice assessment and prescription of assistive technology and home adjustments to CHSP eligible clients.
  • Assist with coordination of a case load planning service delivery and evaluation.
  • Maintain appropriate clinical documentation.
  • Identify and complete referrals to other services as required

THE CANDIDATE

The successful candidate will be a passionate self-driven and innovative Occupational Therapist.

You will be organised and flexible with a high standard of communication problem-solving and initiative. You will be comfortable and capable to work autonomously within the community setting.

You will have sound clinical expertise preferably within the Aged Care sector.

The candidate will possess:

  • A passion for working in community based aged care.
  • The ability to work with individuals care givers health professionals and other staff within a consultative and collaborative environment to achieve positive outcomes for clients.
  • Current registration with AHPRA NDIS Workers Screening check WA Drivers License and current First Aid and CPR.
