About the role

This position is based at Calvarys Launceston hospitals Calvary St Vincents and St Lukes offering the opportunity to work across both sites within a collaborative multidisciplinary environment.

The Discharge Support Liaison is a Full-Time Fixed-Term position until February 2027. The role is primarily worked during standard business hours (Monday to Friday) aligning with peak patient flow and discharge planning activities.

This is a senior non-direct-report role focused on coordination leadership and system improvement rather than line management. You will work closely with clinical teams patient flow and external providers to optimise discharge processes reduce length of stay and improve patient outcomes.

Ideal for an experienced Allied Health clinician Registered Nurse or Care Coordinator this role offers the chance to influence hospital-wide processes build strong stakeholder relationships and contribute to meaningful improvements in patient flow and continuity of care.

Your main duties will include:
  • Lead and coordinate patient-centred discharge planning from admission through to transition of care ensuring safe timely and effective outcomes
  • Collaborate with multidisciplinary teams to reduce length of stay address discharge barriers and improve patient flow across both hospital sites
  • Liaise with patients families and external providers to arrange appropriate supports and ensure clear communication of discharge plans
About you:
  • Current AHPRA registration or relevant credential
  • Experience in discharge planning or care coordination
  • Strong knowledge of hospital systems and patient flow
  • Skilled in communication and stakeholder engagement
  • Ability to manage complex cases and competing priorities
