Job Summary

The Qualified Psychiatric Rehabilitation Practitioner supports individuals living with chronic mental illness (CMI) in reaching their personal goals through the Intensive Psychiatric Rehabilitation Service (IPRS) program. Working under general supervision the specialist applies the rehabilitation technology and philosophy of the Psychiatric Rehabilitation Approach as developed by The Center for Psychiatric Rehabilitation at Boston University.

What you will do as the Qualified Psychiatric Rehabilitation Practitioner:

Clinical & Direct Service

  • Ensures informed consent for IPR services and that all related legal documents and releases of information are completed in accordance with agency policies and procedures.
  • Represent the program at community events and recruit prospective clients.
  • Conducts triage interviews to determine client eligibility for Intensive Psychiatric Rehabilitation (IPR).
  • Completes an initial readiness assessment and service plan upon admission with reassessments conducted based on client progress toward goals.
  • Completes a comprehensive social history within 30 days of admission and updates it at least annually.
  • Develops individualized crisis plans in collaboration with each client.
  • Documents monthly changes in a clients motivational readiness to choose valued roles and environments.
  • Provides all phases of IPR including Triage Readiness Assessment Readiness Development Goal Choosing Goal Achieving and Goal Keeping.
  • Meets with clients individually and in cohort settings focusing on overall recovery for 410 hours per week.
  • Completes prior authorizations for clients to receive IPR services through their designated funder.
  • Transports clients safely and legally within the community as needed.

Documentation & Compliance

  • Maintains clinical records and all required data reports and documentation in accordance with agency policy and legal and regulatory requirements filing all necessary reports in a timely manner.
  • Maintains expected productivity levels and ensures appropriate follow-up with clients either in person or by phone.
  • Upholds all organizational and departmental policies including those related to confidentiality quality assurance safety and infection control.
  • Adheres to the agencys corporate compliance program and maintains the highest level of professional integrity and ethical practice.

Professional Standards

  • Represents the Community Mental Health Center (CMHC) in a professional manner at all times.
  • Attends and actively participates in required staff meetings webinars trainings and in-services.
  • Pursues ongoing professional growth through seminars workshops and professional affiliations.
  • Demonstrates an understanding of and models the organizations mission and core values through behavior and attitude displaying patience tact fairness and respect with staff clients families service providers and the broader community.
  • Remains flexible and receptive to changes in work schedule and assignments.
  • Performs all other duties as assigned.
Requirements

The Qualified Psychiatric Rehabilitation Practitioner must:

  • meet all regulatory requirements for employment at a Community Mental Health Center in the State of Iowa or Illinois as required by state and federal law including successful completion of a criminal background investigation Illinois CANTS clearance (when applicable) and Office of the Inspector General exclusionary screening.
  • hold a valid drivers license with acceptable auto insurance coverage and meet agency qualifications to operate agency vehicles.

Education

  • Bachelors degree with a minimum of 30 semester hours (or equivalent quarter hours) in a human services field including but not limited to psychology social work mental health counseling marriage and family therapy nursing occupational therapy or recreational therapy.
  • Completion of 60 contact hours of Intensive Psychiatric Rehabilitation Service training from a certified instructor.

Experience & Skills

  • Minimum of one year of experience providing services to the population groups served by this role.
  • Proficiency with technology and information systems.
  • Strong communication and interpersonal skills.
  • Professional presentation and demeanor.
  • Ability to thrive in a fast-paced environment and maintain composure under stress.
  • Ability to collaborate effectively with staff and other service providers and to engage sensitively and respectfully with clients and their families.
  • Prior experience in a community mental health center setting preferred.
