Role Description
The Nurse Practitioner - Hospice role is a full-time on-site position located in San Antonio TX United States. The Nurse Practitioner will provide compassionate and comprehensive care to patients with terminal illnesses focusing on pain and symptom management while supporting patients emotional social and spiritual needs. Responsibilities include conducting assessments developing and implementing care plans collaborating with interdisciplinary teams and educating patients and families about hospice care options. The role involves ensuring compliance with regulatory guidelines and maintaining up-to-date patient documentation.
Qualifications

Strong clinical skills in patient assessment diagnosis and treatment planning

Experience in end-of-life care pain management and symptom control

Ability to collaborate effectively with interdisciplinary teams to coordinate care

Excellent communication and interpersonal skills for patient and family education

Masters degree in Nursing and licensure as a Nurse Practitioner (NP) in the state of Texas required

Certification in Hospice and Palliative Care (preferred)
