We are looking for a Family Worker to join our team!
The Family Worker will provide individual family and group support using lived experience strategies to encourage learning and growth. This position will use a stepped care approach to provide consultation and support to the team increasing knowledge and strategies of family systems.
Status: Permanent full-time 37.5 hours per week. Note this posting is for an active vacancy.
Salary: $67419.02 - $79316.51 annually. (Candidate qualifications will be considered when determining the starting salary)
Location: This position is based out of our 60 West Drive Brampton office. Due to the nature of this program this role is on-site and/or community based.
What you will do
- Provide individual and group supports for families
- Complete a family assessment to identify areas of need and develop care plans
- Increase awareness of resources and provide education about mental health and addictions to families to address their concerns
- Support families in building healthy relationships enhancing personal connections implementing self-care strategies and becoming strong advocates
- Facilitate groups as part of CMHA PDs multidisciplinary team including the creation of new groups and act as a back-up facilitator
- Manage a caseload including regular re-assessments updating risk levels and caseload reviews
- Refer and link families to community resources and groups that support their goals
- Participate in regular staff meetings and care planning to provide family systems expertise for client care
- Contact primary care at regular intervals to report clients care plan and discharge summary
- Work collaboratively with other Family Workers to support organizational needs (e.g. volumes across all regions)
- Update all appropriate documentation within 24 hours of client contact as required by CMHA PD policy and procedure.
- Participate in the Family Community of Practice and other agency and community committees and workgroups as requested by manager.
What you need to join our team
- Personal family experience with mental health and/ or addiction as well as a willingness to share experience
- Post-secondary degree or diploma in relevant discipline (e.g. BSW MSW OT RN)
- Must be registered in the province of Ontario as a member in good standing with an appropriate regulatory college (e.g. Registered Social Worker Registered Social Service Worker Registered Occupational Therapist Registered Nurse or Registered Psychotherapist)
- Minimum 2 years of experience in the mental health and addictions sectors with a focus on assessment brief interventions and short term supports
- Expertise in community mental health and/or community based human services
- Demonstrate crisis intervention skills for both telephone and in-person situations
- Utilize trauma informed best practices
- Empathize and connect with those experiencing the mental health and addictions system
- Conduct assessments and coordinate services that are recovery-oriented based on need
- Demonstrate excellent decision making problem recognition and problem-solving skills
- Demonstrate strong computer literacy in Microsoft Office suites virtual platforms and client database management
- Might be required to work evenings and weekends
- Strong interpersonal skills with proven ability to approach listen build trust and communicate across diverse cultures and intersectionality. Willingness to continually improve understanding around cultural competence.
- Openness to operate from an anti-racist anti-oppressive 2SLGBTQ affirming and intersectional framework.
- Willingness to learn and apply practical understanding of different elements of diversity such as gender identity/expression race/cultural background religion sexual orientation disability immigration and precarious status family status etc.
- Fluency in a second language including ASL (American Sign Language) is considered an asset.
- Proof of administered full-series Health Canada approved COVID-19 vaccination prior to start date. If unable to be vaccinated for reasons relating to protected grounds under the Code completed medical or religious exemption must be provided.
- Valid Canadian drivers license and access to an insured automobile in good repair
- A satisfactory Vulnerable Sector Screening (Police Check) report.
