Scheduled Weekly Hours:

The Adolescent Partial Hospitalization Program (PHP) is seeking a licensed social worker to cofacilitate and facilitate therapeutic groups and provide direct support to adolescents ages 1318 during program hours (9:00 a.m.3:00 p.m.). This role involves working closely with youth in a structured teambased setting to support emotional regulation skill development and overall treatment engagement throughout the day. The ideal candidate values collaborative care and enjoys building meaningful connections with adolescents in a groupbased treatment environment.

What you will work:

• This position is full-time (40 hours/week) working primarily day shifts Monday Friday

• Location: Onalaska WI Onsite

What you will do:

• The Adolescent PHP Social Worker functions as an essential part of the treatment team; assessing incoming referrals scheduling appointments coordinating patient care providing or assisting with crisis intervention meeting with patients facilitating/co-facilitating groups and assisting with programming needs.

• This individual will have strong communication skills be task driven enjoy working with adolescents and have an appreciation for a team approach.

• Act as a consultant and leader in comprehensive care throughout the system while supporting organizational strategies and goals.

What you will need:

• Bachelorsdegree in Social Work or in a related field

• WI Social Worker license

• 1 year of experience in human services or health care
