Job Summary:

The Chart Prep Nurse is an integral part of the surgical and procedural team. The Chart Prep Nurse is responsible for organizing and completing the patient chart and conducting the pre-op phone call for scheduled cases. The Chart Prep Nurse works collaboratively with scheduling pre-op the operating room and physician offices to ensure chart completeness and readiness for the day of the scheduled surgery/procedure. The Chart Prep Nurse communicates with the mentioned departments and any others as necessary regarding any changes or special needs to ensure all departments are prepared.

Hours:

  • .75 FTE - 30 Hours Per Week
  • 10-Hour Shifts

Essential Job Functions:

  • Advocate for patients rights preferences and needs ensuring that they receive high-quality compassionate care that is tailored to their individual circumstances
  • Project a professional demeanor and appearance while maintaining the confidentiality of the patient coworkers and the organization as appropriate
  • Review patient medical history current medications allergies and surgical consent forms; identifying potential risks and report abnormalities to the surgical or anesthesia team
  • Assess patient health problems and needs to develop implement and evaluate care in collaboration with healthcare team
  • Ensure infection control protocols and safety standards are followed in all aspects of patient care
  • Pre-emptively calls and gathers information on patients that are scheduled up to two weeks prior to surgery
  • Prepares and coordinates all activities regarding patient chart preparation
  • Prioritizes locates assembles electronically documents and sees to the appropriate transport of records within the department
  • Provide patients and families with pre-operative instructions including fasting guidelines medication management and what to expect during the surgical process
  • Communicates with Provider offices to obtain history and physicals medication lists labs and any other pertinent information needed to appropriately prep a patient for surgery/procedure
  • Identify patients with special needs or medical complexities and alert the clinical team to ensure proper preoperative planning
  • Utilizes critical thinking and communicates with Pre-operative Clearance Committee and providers regarding patients who may need further examination prior to surgery
  • Retrieves and completes add-ons throughout the day to ensure all needs are met
  • Is knowledgeable of the needs for all specialties in the department in order to educate patients during the chart prep phone call
  • Describes the preoperative experience to the patient guardian and family including what to expect before during and after procedure
  • Reports abnormal values to anesthesiologist and/or provider in a timely manner
  • May be cross-trained and assigned to other nursing departments within the hospital with responsibility for carrying out the essential functions of the assigned role
  • Use the organizations secure messaging platform in accordance with departmental protocols including timely receipt and responses to communications
  • Perform other duties as assigned

Knowledge Skills and Abilities:

  • Medical Terminology
  • Electronic Medical Records (EMR) systems
  • Quality assurance and accreditation standards
  • Understanding of HIPAA regulations patient confidentiality laws and patient rights
  • Effective English communication in verbal and written format
  • Quality work including attention to detail and accuracy
  • Basic computer proficiency
  • Carry out customer service standards
  • Prioritize tasks effectively through managing workload based on urgency and importance
  • Capacity to work independently and as part of a multidisciplinary team
  • Continuously learn and improve through staying up to date on job specific trends policies and new technologies
  • Successfully demonstrate organization-wide performance review competencies

Qualifications:

  • Demonstrates eligibility for employment in the U.S.
  • Associate Degree in Nursing (ADN) from an accredited nursing program required
  • Current RN licensure in the State of Wisconsin
  • BLS certification is required upon hire
  • Previous perioperative or general nursing experience is preferred
  • Must possess a cell phone that interfaces with the organizations secure messaging system
