LOCATION: St. Marys Campus - 89 Genesee St. Rochester NY 14611

DEPARTMENT: Physical Therapy Rehabilitation

SCHEDULE: Day Shift 8-hr shifts 8 am - 4:30 pm; weekend rotation a

minimum of one shift a month is required

POSITION SUMMARY
Assesses and restores patients function alleviates pain and prevents disabilities by designing planning and implementing medically prescribed physical therapy interventions. Provide superior customer service by modeling the Brand Promise and Core Values.

RESPONSIBILITIES

• Treatment & Planning. Meet patient goals and needs by providing quality evidence-based care; assess develop and implement an individualized treatment plan; help the patient accomplish the treatment plan and accept therapeutic devices by administering manual exercises; instruct encourage and assist patients in performing physical activities such as non-manual exercises ambulatory functional activities and daily-living activities

• Discharge Management. Assure continuation of therapeutic plan following discharge by designing home exercise programs; instruct patients families and caregivers in home exercise programs; recommend and provide assistive equipment; recommend outpatient or home health follow-up programs

• Leadership. Develop physical therapy staff by providing information; develop and conduct in-service training programs

REQUIRED QUALIFICATIONS

• Masters Degree in Physical Therapy or Bachelors degree with prior experience as a licensed Physical Therapist required.

• Required Licensure/Certification: Current New York State license or a limited permit as a Registered Physical Therapist.

• If a New York State license or limited permit is not held the applicant must be hired within 90 days of graduation from a New York State or APTA-accredited physical therapist education program and meet the Practice Exemption under New York State Physical Therapy License Requirements.

• Exceptional communication and documentation skills

PREFERRED QUALIFICATIONS

• Prior Physical Therapist experience preferred.

PAY RANGE:

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience relevant qualifications specialty internal equity location and contracts.
