Are you passionate about supporting others and making meaningful connections Glacier Hills is seeking a dedicated Full-Time Social Worker to join our Social Services Department and play a vital role in enhancing the lives of our residents and their families.
About the Role
As a Social Worker at Glacier Hills you will be an advocate resource and trusted partner in care. Your responsibilities will include:
- Participating in discharge planning resident assessments and the development and implementation of individualized social care plans
- Conducting cognitive and mood assessments and coordinating appropriate follow-up care and treatment
- Completing timely and accurate documentation including medical forms reports evaluations and charting
- Referring residents and families to community resources and appropriate social service agencies
- Educating residents and families on Medicare Medicaid and other financial assistance programs
About You
You are compassionate organized and committed to resident-centered care. Qualified candidates will have:
- Active Social Worker licensure in the state of Michigan
- A Bachelors or Masters degree in Social Work or a related human services field (such as Sociology Psychology Special Education or Rehabilitation Counseling)
- One year of supervised social work experience in a healthcare setting (preferred)
- Experience working in long-term care or with the geriatric population (preferred)
