Were seeking a Case Manager to guide patients and families through the rehabilitation journey ensuring seamless communication effective care planning and smooth transitions.

How youll contribute:

• Lead interdisciplinary care coordination and discharge planning
• Identify barriers to progress and help develop solutions with the care team
• Document care activities and complete the Individual Plan of Care (IPoC) per CMS guidelines
• Facilitate weekly team conferences and family meetings
• Communicate patient progress and discharge plans to caregivers
• Review Case Mix Groups (CMGs) and collaborate with HIM on comorbidities
• Support additional departmental needs as assigned

Qualifications and requirements:

• Active licensure as an RN Social Worker Respiratory Therapist PT OT or SLP
• 2 years of case management or social work experience in an inpatient setting (rehab experience strongly preferred)
• Strong communication organization and collaboration skills
• Proficiency in Microsoft Office
• Case Management or Rehab Nursing certification (CCM ACM RNBC ARN); preferred
