Job Summary

Under minimal supervision the Supervisor of Care Management is responsible for providing operational direction supervision and oversight to inpatient care management teams that perform the functions of utilization review and management progression of care facilitation and transition of care coordination. Participates in planning implementing and managing existing and new Care Management programs policies and procedures. Ensures compliance with all policies and procedures as well as compliance with all state federal regulatory and payor requirements. Directs patient assignments and staff scheduling according to census demands and assists in meeting utilization financial quality and patient satisfaction targets by effectively assisting in managing utilization productivity personnel and supplies.



Essential Functions:

  • Supervises the daily activities of the assigned employees providing guidance and direction as needed
    Makes staff assignments to ensure coverage to all areas
    Conducts daily rounds to provide support for decision-making and problem solving
    Serves as a resource to the healthcare team regarding inpatient care management issues

  • Participates in the development refinement implementation and administration of Care Management programs policies procedures and standard work
    Provides daily staff oversight and direction on Care Management processes to ensure compliance with all departmental policies and processes
    Coordinates ongoing staff education and training regarding current and new programs and processes

  • Monitors and evaluates compliance with performance standards for each key process on a regular basis
    Monitors staff activity and productivity
    Assists in developing performance reports
    Analyzes patterns of variance and implements strategies to improve departmental/staff performance
    Provides feedback information to the staff regarding departmental and individual staff performance
    (10%)
  • Responsible for effective collaboration with internal and external resources to achieve goals and objectives
    Leads productive meetings with interdisciplinary team members (physicians residents mid-levels nursing other healthcare professionals and external providers etc.)
    Develops and initiates cost saving strategies to achieve decreases in resource utilization and patient throughput
    Participate on committees and workgroups to develop collaborative and integrated work processes

  • Ensures compliance with state federal regulatory and payer requirements
    Ensures the application of medical necessity criteria to support appropriate status placement level of care and transition of care processes
    Ensures that payers receive the clinical reviews necessary for admission and continued stay day certification
    Implements monitoring plans for the Important Message from Medicare and patient choice and other regulatory and payor requirements

  • Provides staff management activities
    Participates in the hiring orienting training performance reviews counseling disciplining and terminating of employees

  • Assists in preparing and maintaining the departmental budget
    Ensures confidentiality of patient information by creating and maintaining a secure and trusting environment by not sharing information learned on the job except when necessary in the performance of the job responsibilities or to improve a patients care.

  • Sets clear expectations for staff by developing job specific customer service expectations and behaviors for each position supervised; clearly communicates expectations to each employee and establishes the expectations as job requirements and performance appraisal components; delegates responsibility/authority whenever possible.

  • Identifies training and staff development needs of employees and ensures appropriate training provided. (5%)
  • Actively identifies problems and opportunities for improvement; identifies appropriate solution and involves others in the implementation process. (5%)


Education:

  • Masters Degree Required


Work Experience:

  • Typically five or more years of relevant clinical experience with diverse populations in a healthcare setting including hospital clinic or home health providing patient education case management and/or care coordination. Required and
  • 1-3 years Experience in discharge planning home health care community health or managed care Required and
  • Two or more years supervisory experience preferred.


Licenses:

  • BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association or American Red Cross Required and
  • LCSW-Licensed Clinical Social Worker - LCSW State Licensing Board Required


Knowledge Skills and Abilities:

  • Strong program development leadership and management skills.
  • Knowledge of care management concepts principles strategies and processes.
  • including tools and techniques for identification stratification and management of high-risk clients.
  • Demonstrated ability for assessment evaluation and interpretation of medical information and care planning.
  • Possess a high level of understanding of community resources treatment options funding options and special programs.
  • Extensive knowledge of the management of acute and chronic conditions.
  • Ability to operate PC-based software programs including proficiency in Word Excel and PowerPoint and other computer-based systems.
  • Possess advanced written and oral communication skills decision making and problem-solving skills.
