HomeLife Personal Care is in search of an experienced Home CareRN to perforn SOC assessments VA Assessments and supervisory visits on a PRN basis. HomeLife Personal Care is a nurse owned licensed non- skilled Home Health agency providing Aides and HHAs to the homes of our clients. We have been proudly providing care to the region since 2017 and we are seeking someone who would be a great fit with our team
Responsibilities
Visit patients in their homes for one-on-one Assessments and Supervisory Visits
Complete initial and ongoing assessments to identify the physical psychosocial and environmental needs of home health patients
Assess patients conditions and chart observations and perform evaluation tasks including medication review
Educate patients and their families on proper home health care strategies and procedures and make recommendations for devices or tools that might improve the patients quality of life
Develop a Home Health Aide Plan of Carebased on the assessment findings.
Provide effective communication to patients/clients their family members team members and other health care professionals
Participate in weekly team meeting via Zoom or in Person
Qualifications
Graduate of an accredited Diploma Associate or Bachelors School of Nursing
Current state license as an RN
Proof of current CPR certification
Compassionate friendly demeanor
Excellent communication skills
Current drivers license and reliable transportation
Ability to work a flexible schedule and travel locally
