Job Summary
Job Title: Travel LPN / LVN Long Term Care (LTC)
Location: Glenwood Minnesota
Shift: Day Shift 40 Hours/Week
Assignment Length: 9 Weeks
Work Setting: Onsite
Pay Rate: $35/hour on W2
Position Summary
We are seeking a dependable and compassionate LPN/LVN for a 9-week assignment in a Long-Term Care (LTC) and Rehabilitation setting. The ideal candidate will have prior LTC experience and the ability to provide quality patient care in a fast-paced healthcare environment.
Key Responsibilities
- Provide direct nursing care to long-term care and rehabilitation patients
- Administer medications and treatments as prescribed
- Monitor and document vital signs and patient health status
- Perform blood glucose monitoring and related patient care
- Assist patients with activities of daily living while ensuring safety and comfort
- Maintain accurate documentation within EMR systems
- Collaborate with physicians RNs and interdisciplinary healthcare teams
- Float between facility units as needed
- Follow infection prevention and patient safety protocols
- Respond appropriately to changes in patient condition
Required Qualifications
- Active LPN/LVN license required
- Minimum 1 year of LTC experience required
- Current BLS Certification required
- Experience in Skilled Nursing Facility or Nursing Home settings preferred
- Familiarity with EMR systems; Epic experience preferred
Skills & Competencies
- Strong clinical and patient care skills in LTC settings
- Experience with blood glucose testing and monitoring
- Ability to manage higher patient ratios effectively
- Excellent communication teamwork and documentation skills
- Flexibility to float between units as needed
