In This Role You Will:
Provide compassionate patient-centered hospice nursing care
Develop and implement individualized plans of care in collaboration with patients caregivers providers and the interdisciplinary team
Educate patients and families about hospice philosophy services and care expectations
Perform ongoing assessments and document patient progress interventions and outcomes accurately
Partner closely with physicians healthcare professionals hospice team members and volunteers to coordinate care
Support patients and caregivers through meaningful education and emotional support
Participate in interdisciplinary team meetings quality initiatives and agency programs
Contribute to a culture of continuous learning and excellence in care

Requirements
Graduate of an accredited school of nursing accompanied by current New Hampshire nursing license or compact state license through the Nurse Licensure Compact (NLC).
Minimum of two years acute care nursing experience.
Three to four years of recent general medical/surgical or skilled/acute care nursing experience.
