In This Role You Will:

• Provide compassionate patient-centered hospice nursing care
• Develop and implement individualized plans of care in collaboration with patients caregivers providers and the interdisciplinary team
• Educate patients and families about hospice philosophy services and care expectations
• Perform ongoing assessments and document patient progress interventions and outcomes accurately
• Partner closely with physicians healthcare professionals hospice team members and volunteers to coordinate care
• Support patients and caregivers through meaningful education and emotional support
• Participate in interdisciplinary team meetings quality initiatives and agency programs
• Contribute to a culture of continuous learning and excellence in care

• Graduate of an accredited school of nursing accompanied by current New Hampshire nursing license or compact state license through the Nurse Licensure Compact (NLC).
• Minimum of two years acute care nursing experience.
• Three to four years of recent general medical/surgical or skilled/acute care nursing experience.
