Job Title: Licensed Practical Nurse (LPN) Long-Term Care (LTC)

Location: Glenwood MN
Duration: 9 Weeks

Schedule

• Shift: Day Shift
• Hours: 5x8-hour shifts (40 hours/week)
• Working Days: MondayFriday

Position Summary

We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) with Long-Term Care (LTC) experience to provide high-quality care in a skilled nursing environment. The ideal candidate will demonstrate strong clinical skills effective communication and a commitment to resident-centered care.

Key Responsibilities

• Provide direct patient care including monitoring vital signs and administering medications
• Assist residents with activities of daily living (ADLs) such as bathing dressing and feeding
• Monitor document and report changes in patient conditions to RN or physician
• Administer treatments and perform wound care as ordered
• Maintain accurate and timely patient documentation
• Collaborate with interdisciplinary teams to develop and implement care plans
• Educate residents and families on health conditions and care plans
• Maintain a safe clean and comfortable environment
• Follow infection control and safety protocols

Required Qualifications

• Active LPN license
• Experience in Long-Term Care (LTC) or Skilled Nursing Facility (SNF) preferred
• Strong clinical and patient care skills
• Excellent communication and teamwork abilities
• Ability to work in a fast-paced healthcare environment

Preferred Qualifications

• BLS/CPR certification
